Job Title: Nurse Practitioner
Employment Type: Independent Contractor, Part Time / Full Time
Position Overview:
The Nurse Practitioner (NP) at Connect Health is a key member of the medical team providing outstanding preventive medical care and promoting longevity medicine for patients. The NP provides primary care services to patients, focusing on comprehensive patient care, preventative medicine and overall wellness. In collaboration with the Connect Health clinical care team, the NP also actively participates in administering Connect Health longevity medicine programs. As required, the NP may assist with nursing responsibilities, including patient intake visits, phlebotomy, IV therapy, and other duties as required.
Key Responsibilities:
- Provide comprehensive care to patients for a wide range of health needs, including routine checkups, managing chronic conditions, and providing preventative care.
- Conduct comprehensive patient assessments, including medical history, physical examinations, and diagnostic evaluations as part of Connect Health longevity medicine programs.
- Order and interpret laboratory tests, imaging, and other diagnostic studies as part of Connect Health longevity medicine programs.
- Build long-term relationships with patients, understanding their individual needs and health history.
- Maintain accurate and up-to-date electronic health records (EHR).
- Stay informed about emerging research and advancements in longevity medicine and preventive healthcare.
- Cover nursing responsibilities as required.
Qualifications:
- Licensed Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
- Strong interest in longevity medicine, preventative healthcare, and wellness-focused treatments.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively in a dynamic healthcare setting.
- Basic proficiency in electronic health records (EHR) systems.
Preferred Qualifications:
- Basic knowledge of integrative medicine, functional medicine, or longevity-focused healthcare (training will be provided as needed).
- Experience in primary care, internal medicine, or wellness-oriented practice (preferred but not required).
Salary and Compensation:
- Base compensation starting from $100 / hour; ~35 hrs / week

Position Overview
As a Client Care Coordinator, you will be the first point of contact for clients and guests, ensuring they experience the excellence Connect Health is known for. You will educate new and prospective clients about our range of programs, address inquiries, and ensure smooth onboarding into our unique care model. You will also play a significant role in both the ownership and management of the individual patient experience. The mission of a Client Care Coordinator is to ensure exceptional organization is behind each patient experience, which drives the operational success. This includes things like scheduling, task assignment and completion, patient communication, and handling specialist referrals. Through exceptional communication and sophisticated delivery of patient care, you will become an invaluable part of each and every patient’s longevity journey.
Key Responsibilities
Client Experience & Communication
- Serve as a trusted point of contact, delivering seamless, concierge-level service across all platforms.
- Personalize support based on each client’s unique health and wellness goals.
- Handle scheduling, inquiries, payment processing, and special requests with professionalism and care.
- Provide information to new and prospective clients about Connect Health programs and services.
- Act as a brand ambassador by consistently reflecting Connect Health’s mission and values.
- Foster trust, loyalty, and satisfaction through every client interaction.
Patient Care Coordination
- Own and manage the operational aspects behind each patient experience, including scheduling, task assignment, and patient communication.
- Coordinate patient visits to drive progress and ensure continuity of care.
- Manage and track the patient journey, ensuring deadlines and key events are monitored and communicated.
Administrative Support
- Create, maintain, and organize EMR records, patient results, and patient-facing documents.
- Retrieve, manage, and organize patient medical records and specialist referrals.
- Work closely with Physicians to ensure thorough preparation for patient encounters.
- Communicate directly with external care teams (assistants, specialists) to facilitate appointments and resolve requests.
- Ensure data organization for laboratory results, imaging, and clinical documents, presenting them to physicians for review.
Qualifications
Education
- Bachelor’s degree in a relevant field (Science, Hospitality Management, Health Administration, or a related discipline), or completion of a recognized Medical Office Assistant certificate or Health Administration diploma program (or equivalent formal training).
Experience
- 3–10 years of experience coordinating patient care in a clinical or hospital setting.
- Previous exposure to high-touch, customer/patient service environments.
- Familiarity with healthcare operations and electronic medical records (EMR) systems strongly preferred.
