Medical records can only be released with your authorization. A Medical Records Release form is available from Client Services and can be filled out should we need to obtain your previous records. You are responsible for obtaining relevant medical records from other physicians or health-care providers. Please contact them and allow sufficient time for your request to be completed.
As a non-profit practice, practitioners do not make any money from the recommendations of advanced testing, supplements or referrals to other practitioners within the clinic. The proceeds go back into our clinic to improve the quality of care and service. At Connect Health, we strive for the highest ethical standards and want to ensure that there is no conflict of interest in our recommendations.
You will receive a reminder from Client Services the day before your scheduled visit. In general, we require 24 hours’ notice to cancel or change any appointments. Because we are a small clinic and time has been held just for you, we are affected by last minute changes.

Additionally, because appointments are in high demand, a last minute cancellation prevents someone else from taking your spot who may have benefited from the treatment or consultation time. Thank you for understanding the need for this policy.

If we don’t receive adequate notice of cancellation, you will be charged for the full amount of a missed, no-show or last minute cancellation.

Cancellations of Enhance appointments within the 24-hour cancellation time frame will be assessed a $35.00 charge.

We require three business days’ notice should you need to change or cancel your Begin appointment, otherwise your credit card will be charged for the full amount of your visit. This is necessary due to the limited number of Begin appointments available and the number of patients waiting.

There have been updates to the Laboratory Services Act as of October 1, 2015 which govern publicly-funded lab services.

As a result of this update, preventative medical lab services are not considered billable under MSP. This means that any referrals for such tests your physician may request, will need to be paid for privately.

However, should you have a medically indicated health concern, your physician is able to bill MSP for any laboratory or diagnostic tests they think are warranted and you will not be assessed any fees.

We are happy to accommodate your request for a prescription refill under the following guidelines:

This service is available to our patients at a charge of $25 per renewal (there is no charge if you are an Enhance patient). This covers the administration time of both physician and Client Services to arrange with your pharmacy.

If you have not been to Connect Health in over six months, your physician may request a follow-up visit prior to refilling a prescription. You may need updated lab work and a review to ensure that the prescription and dosage are still suitable for your present needs.

Please allow at least three business days for us to process your request as we cannot readily assist with more urgent refills. As such, we encourage you to monitor your medication and review your needs with your physician during your medical appointment.

Requests for medical records and test results can only be made by patients directly and a properly signed Medical Records Release form needs to be completed. This procedure will ensure the privacy and confidentiality of all medical records. Please note that emailing or photocopying of records or lab results is subject to a fee.
